For the physically handicapped
- a ZTP/P card and statement from the relevant local authority acknowledging an exceptional level III disability and proving that the handicap necessitates the issuance of a ZTP/P card,
- a ZTP card and statement from the relevant local authority acknowledging an exceptional level II disability proving that the handicap corresponds with a handicap mentioned in paragraph 2, letter i) of Appendix no. 2 to Notice no. 182/1991 Coll., as amended (complete or partial deafness); if the statement does not include the specific type of physical handicap, then a doctor's certificate confirming complete or partial deafness of the claimant,
- if the claimant is dependent on the help of another person, a statement or notification acknowledging a subsidy for care,
- furthermore, in the case of claimants under 18 years of age, or who are under the care of a guardian (or whose capacity to enter into legal transactions has been otherwise restricted),

- court confirmation regarding guardianship or restriction of the claimant's capacity to enter into legal transactions or in regard to legal representation,
- the parent or guardian should have a birth certificate for the claimant (or the claimant must be registered in the parent or guardian's ID or passport)
If a claimant requesting admission of a claim refuses to photocopy documents, they must submit a Statutory Declaration with their request.
If the request is submitted in writing, certified copies of documents need to be submitted, including a signature on a Statutory Declaration. If a certified copy cannot be made for legal reasons, a normal copy of the document may be submitted.
A claimant can be represented by another party when submitting the request if that party has been given Power of Attorney.
