- SIM card Kombi is a new type of SIM card, enabling the use of 2 independent telephone numbers/accounts. These separate telephone numbers/accounts are referred to as identities A and B.
- Identity A may be allocated only on the basis of a written Contract concerning the Provision of Telecommunication Services (hereinafter the "Contract").
- Identity B may be allocated on the basis of a Contract signed with O2 or to users of O2's telecommunication services through a SIM card with prepaid services ("O2 Go").
- The client with identity A is entitled to make use of the services offered by SIM card Kombi and dispose of the SIM card Kombi, upon the payment of the SIM card Kombi Activation Fee. The price for activation of the SIM card Kombi shall be based on the currently valid Pricelist.
- The client with identity B is entitled to make use of the services offered by the SIM card Kombi only with an approval of the client with identity A. This will be the case from the time of transfer or connection of identity B to the SIM card Kombi until any future separation from the SIM card Kombi.
- After identities A and B are transferred from separate SIM cards to the SIM card Kombi, these separate SIM cards will be deactivated and the clients with identities A and B will be able to make use of any further services only through their SIM card Kombi, to which these identities are transferred (connected).
- All fees pertaining to the use of the SIM card Kombi will be charged to the client with identity A. This concerns particularly the price for the activation of the SIM card Kombi, the monthly tariff for the use of the SIM card Kombi, and fees pertaining to the replacement of the SIM card Kombi in case of theft or loss, etc.
- A monthly tariff is charged for the use of the SIM card Kombi, according to the valid Pricelist.
- Pursuant to a written request submitted by the client or the user with identity A or B, one of the identities on the SIM card Kombi may be transferred (separation of SIM card Kombi) to a separate SIM card. The transfer (separation) of identities from the SIM card Kombi can be carried out only in official O2 Praha retail centers or by selected sales representatives of O2 Praha. For this purpose, it is necessary to submit the original SIM card Kombi and prove the identity of the client or the user. The transfer of an identity to a new SIM card will be charged as a replacement of a SIM card, according to the valid Pricelist.
- After separation of identities from the SIM card Kombi, this SIM shall have the characteristics of an independent SIM card. The identity separated from the SIM card Kombi will be deactivated from the SIM card Kombi and therefore cannot make use of any of its services. An identity once deactivated cannot be reactivated.
- SIM card Kombi does not enable the use of certain standard functions available in independent SIM cards, e.g. call forwarding (with the exception of unconditional forwarding), replacement of SIM card Kombi by Duo SIM card, and redirecting SMS messages between identities.
- O2 is entitled to make any changes to any terms and conditions stipulated herein. The current text of the Terms and Conditions pertaining to the services of the SIM card Kombi is available at official O2 retail centers.
- By activating the SIM card Kombi, the clients holding identities A and B express their consent with the conditions under which the SIM card Kombi services are provided.
- These terms and conditions become effective on April 1, 2003.
