Switching subscribers

To switch subscribers you need to fill in the Setting up a Phone Line Transfer form (in Czech only) and send it, along with the documents listed below, to our Customer Care Center at

You can also fill in the form and drop it off at one of our O2 Shops or you can use the relevant online form at the Customers' Portal. If you are unsure about how to fill in the form, our operators will be happy to help you, simply call our toll free line at 800 111 777.

Required documents

Some of the documents listed below must be presented together with the Setting up a Phone Line Transfer agreement.

  • a copy of an identification document (ID, passport); if you submit the request personally at an O2 Store, our employees' verification of the data suffices
  • foreign residents must present the document they use to prove their identity in the Czech Republic (this can be a passport, a diplomatic passport, or a business or a different form of a passport issued by a foreign country; citizens of the Slovak Republic can use their Slovak identification cards) and a permanent or long-term residency permit
  • if you are an entrepreneur you will have to bring your trade license (an abstract from the Trade Registry no older than three months) or another license that permits you to do business - unless you are already an O2 customer
  • if you are a legal entity - an abstract from the Commercial Registry or another registry, or documents permitting you to do you business; these documents must not be more than three months old - unless you are already an O2 customer and there have not been any changes to your data (if the above-mentioned document indicates that more than one person is authorized to deal with this matter as a whole, all of these people must sign the contract)
  • a document confirming that they can use the premises or a property (lease agreement, flat certificate, or a decree about the allocation of residential and non-residential rooms or a decree or an abstract from the Land Registry), where the phone line is located

Additional documents

In special cases the transfer must be substantiated with additional documents:

  • if the switching of subscribers is required by a legal entity or an individual entrepreneur - a document confirming that the rights and obligations of the transferred, merged, or abolished company, or a different subject, that used to be the subscriber to the main phone line were passed to them and that, at the same time, they are the legal user or owner of the premises or property where the main phone line is located.

When a subscription is transferred, the lease of the end-user device ends, along with the use of additional services by the original subscriber, and the reason for disconnection is provided at their own discretion.
